Theory of change (ToC). ToC depicts the various components of the implementation, the assumed pathways through which these components could bring about the targeted changes (mechanisms of change) and the underlying hypotheses that need to hold true for that changes to occur. Indicators are collected at the facility level (weekly reports and monitoring reports) and individual level (cohort study)
