  1. LBW infant defined as delivery of any infant weighting less than 5 ½ pounds at birth. Breastfeeding defined as breastfeeding any infant for 1 month or more. CVD deaths defined as deaths with heart disease (ICD I00–I09, I11, I13 and I20-I51) or cerebrovascular disease (ICD I60-I69) identified as the leading cause of death. CVD deaths + surrogate CVD defined as CVD deaths and deaths with diabetes or hypertension identified as a secondary cause of death
